Required Athletic Medical Forms

We are currently transitioning to a new electronic medical records system. Medical forms are not available at this time. Instructions for completing your required paperwork will be sent to your Randolph College email in June. Please keep an eye on your email for additional information.

ADHD Documentation (FOR ALL STUDENT-ATHLETES)
Any student-athletes taking prescription ADHD medications MUST read the attached letter and fill out the attached ADHD form below.
