Athletic Medical Forms are now available. Please use the following Student Health & Wellness Portal Guide to complete the necessary forms and information for athletic clearance. This guide contains the link to the Student Health and Wellness Portal. All athletic medical information must be completed prior to the first day of practice or you will not be cleared to participate.
